Coach Preseason Checklist:
- Have you ever coached Club ball/were you IMPACT certified previously?
YES: Go to Question #2
NO: Write in an email (do not send it yet) to Ryan saying that you need to get IMPACT certified. Then Go to Question #2
- Have you had a background check completed by USAV either this year or last year?
YES: Go to Question #3
NO: Write in an email to Ryan saying that you need to have a background check done (do not send yet) Go To The USAV GL region website and find the forms for Background Screening Consent. Print and fill out. The link at the bottom should help. Then proceed to Question 3.
I don’t know: Go to the region website and look it up.
- Have you signed the Coaches Code of Ethics this year?
YES: Go to Question #4
NO: Write in an email to Ryan saying that you will deliver a signed Coaches Code of Ethics to Ryan & Go To “Forms” Below. Follow Directions for Printing out Coaches Code of Ethics then proceed to Question 4.
- Are you volunteering or will you be applying for paid positions?
YES, I am volunteering: Write in an email to Ryan that you are volunteering and that you are going to send Ryan a Volunteer Consent Form & Volunteer Waiver & Release Form, then go below to fill out both of those forms. After that, skip Question 5 & go directly to #6.
I will contract for paid assignments: Go to Question 5
- Have you printed out and signed the Great Lakes Individual Membership Form?
YES: Go to Question #6
NO: Write in an email to Ryan saying that you will deliver a signed/filled out Individual Membership Form & Go To “Forms” Below. Follow Directions for Printing out Individual Membership Form then proceed to Question 6.
- Have you filled out the Diamond Elite employer information section & W2?
YES: Go to Question #7
NO: At some point, this will be on Diamond Elite’s website, but until then, Write in an excel document in a horizontal row your information. Each piece of new information should be in a different cell. The information that should be included is: Last name, First name, SS#, Birth date, Mailing address 1st line, Mailing address 2nd line, best contact #, email address, Preferred top size (sweatshirt or jacket), Preferred Shirt size, Preferred bottom size, gender. Attach that excel document to an email to Ryan. Then go to Question #7
- Have you used Diamond Elite’s website to post your coaching bio OR have you sent your bio to Ryan already?
YES: Go to Question #8
NO: Go To “Coach’s Bio” Below. Follow Directions for Coaches Bio then proceed to Question 8.
- Have you sent an email to Ryan listing the previous “No’s” ?
YES: Go to Step #9
NO: Send an email to summers71@hotmail.com listing your status on the previous questions. Deliver any paperwork to Ryan Summers, 1350 N. Arbogast, Griffith IN. Then go to Step 9
- Did you have any forms that need to be received by Ryan?
YES: Call/Email Ryan 219.613.3301 and ask him if he has received all of your paperwork. Repeat step 9 every 2 days until Ryan’s answer is YES, then go to Step 10
NO: go to step 10
- Congratulations, you should be ready for the upcoming club year. Please view the available postings.
For the following, you will need Acrobat Adobe to print off forms
Background Screening Consent Form: G o to www.greatlakesvolleyball.org - then publications – then forms – then juniours. Fill out the first 2 pages.
Coaches Code of Ethics: Go to www.greatlakesvolleyball.org - then publications – then forms – then juniors . Make sure you sign it.
Volunteer Consent Form & Volunteer Waiver & Release Form (2 forms): Go to www.greatlakesvolleyball.org - then publications – then forms – then juniors .
Individual Membership Form: Go to www.greatlakesvolleyball.org - then publications – then forms – then juniors. Make sure you sign it on the back. Do not fill out the middle section. Do not attach any money. There are directions online for filling it out if you need help.
Coaches Bio: Either go to www.diamondelitevba.com and follow the instructions for posting your coaches Bio (page not up yet) OR write your coaching History & a short philosophy statement on a word document and attach it to an email to Ryan. This information will go on the website, so please make it look professional (i.e.; Proofread)
